Why are they called fifth wheels?

The hitch or coupling that connects fifth wheels to a towing vehicle is the shape of a wheel. With many units having tandem axles with four wheel the hitch becomes the fifth wheel. Interestingly the hitch itself is commonly know as a fifth wheel too. Similar coupling on semi trailers are also known as fifth wheels.
